The Role

Bolton Council is seeking to recruit an additional Social Worker to join the Integrated Discharge Team at the Royal Bolton Hospital.

This post provides an ideal opportunity for a qualified Social Worker to join a busy hospital-based team, consisting of staff drawn from nursing and social care backgrounds, to assist in arranging timely and safe discharges from hospital. You will have an opportunity to contribute to the ongoing development of innovative ways of working across health and social care in order to improve the experience of service users and their carers, as well as promote their well being and independence. Applications from newly qualified Social Workers or those about to qualify will be considered.

The ability to develop, maintain and improve relationships with service users and their carers is essential, as is a strong commitment to effective multi-disciplinary team working.

About Us

In this role, you will undertake assessments of need under the Care Act 2014, prepare person-centred care and support plans and also link people to universal and voluntary services. You will work closely with health and social care colleagues as necessary, especially members of the hospital-based multi-disciplinary teams, and provide support with complex cases, including mental capacity and best interest assessments.
