Placing adults at the heart of our work

We are looking for a Social Care Assessor who shares our values and commitment and are passionate about improving the lives of our local residents.

We currently have a Social Care Assessor vacancy within our Hospital Social Work team in Richmond. The team’s Social Care Assessors play a crucial role in assessing and reviewing the social care needs of adults with a focus to provide a holistic, personalised service to residents over the age of 18 who have physical disabilities and older people with mental health disabilities. We would like to hear from people who are passionate about delivering the best services to eligible adults.

Your duties will include:

* undertaking assessments of individual and carers needs and risks to encourage independence, provide support and prevent harm.

* working in partnership with adults service users and carers using a person-centred approach, with the aim of maximising independence, wellbeing and choice
