The City of York Council are looking for dynamic, motivated and creative social care practitioners to be part of our new Transforming Care/ Learning Disability Team.

About the role;

You will work closely with multi- agency partners to support young people and adults with a Learning Disability and/ or Autism who may be at risk of admission to hospital.

Early identification of behaviour that challenges and may cause hospital admission is key to the role and an ability to work to work creatively with partners. Similarly in the event of admission to hospital the role will be to expedite safe and effective discharge from hospital to the community.

You will work with this cohort of people in the community who are on the Dynamic Support Register in a preventative and co-ordinated way with our partners across the city of York to secure and ensure stable accommodation for them.

Key skills and experience required;

  • You will be a qualified social worker, occupational therapist or nurse.
  • You will have experience of working with young people ad adults with a Learning Disability and / or autism.
  • You will have some knowledge of the Mental Health Act including the Forensic Sections of the Act and will have knowledge of the Deprivation of Liberty Safeguards, the Mental Capacity Act, the Court of Protection and the interface between them.
  • You will have an excellent working knowledge of the Care Act and experience of providing robust strengths based assessments which have a focus on the local community and an asset based approach.
  • You will have a good knowledge of Continuing Health Care and Section 117 Aftercare.

2 full time posts and 1 part time post available. 3 year Fixed term contracts.

As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C2: Can express him/herself spontaneously at length with a natural conversational flow, avoiding or backtracking around any difficulty so smoothly that the person with whom they are conversing is hardly aware of it.
