You will work within the MOSAIC Integrated Service for Children with Disabilities alongside health and social care colleagues, taking on referrals to support disabled children and their families during and after assessment by health professionals. The post holder will support families with children aged from 0 to 18 years but will predominantly be based within the MOSAIC Child Development Team who work with children under 5yrs. The post holder will ensure a coordinated team around the child approach and will be responsible for an allocated caseload of disabled children with complex health, educational and social care needs ensuring the coordinated delivery of a planned package of services. You will build positive relationships with parents, using a strength-based approach and will contribute to multi-disciplinary and multi-agency assessment.
This post is exempt from the Rehabilitation of Offenders Act and will require an Enhanced DBS.

You will have knowledge and understanding of the pressures faced by families with children living in socially deprived neighbourhoods with the issues and needs of disadvantaged families with children; including discrimination and in particular the issues faced and needs of disabled children and their families. Experience of working with children with disabilities and of providing support to disabled children and their families is essential.
You will have work experience with children and their families ideally in a Social Care, Health or Education setting with some experience of undertaking assessments of individual needs and/or risk, designing and implementing care/transition plans and monitoring and reviewing such plans over a period of time.
